Clinical Support Coordinator Associate - Monterey Park, CA
Day to Day Responsibilities: Clinical Support Coordinator
  • Performs telephonic and written provider communication to support treatment of the member for BHT.
  • Ensures that approved treatment plans are used as developed and requests modifications to the treatment plan as appropriate, in consultation with the Board Certified Behavior Analyst (BCBA). 
  • Ensures the Provider (via the treatment plan and the results) is addressing both long and short-term goals, and regularly assesses the progress or lack thereof of the member’s progress, in consultation with the Board Certified Behavior Analyst (BCBA).
  • Identification of barriers to meeting goals or complying with the plan.
  • Development of a schedule for follow-up and communication with the provider.
  • Development of a schedule for follow-up and communication with the family and/or caregiver
  • Process to assess progress against the BHT case management plans for members, in consultation with the Board Certified Behavior Analyst (BCBA).
  • Communicates collaboratively with all healthcare professionals involved in the members care to achieve expected outcome(s).
  • Generates referrals to contracted ancillary service providers and community agencies in collaboration with the member’s provider.
  • Documents in the designated Case Management Database and authorization systems when applicable.
  • Arranges and participates in multi-disciplinary patient care conferences.
  • Monitor, document, and report pertinent clinical data as established per UM policy and procedure.
  • Generates referrals as appropriate for BHT services.
  • Enters data in multiple systems as necessary for the generation of reports related to BHT case management.
  • Conducts case review with the BCBA Case Manager, or subsequent superior, as appropriate.
  • Performs other duties as may be assigned by direct supervisor or manager.

- Schedule: M-F 8-6PM. Needs to make sure there is coverage for the providers during their hours
